  • New Patients – if you are interested in setting up a new patient appointment you may do so by using this form, asking your physician for a referral, or calling the office directly.
  • Current Patients – if you have a non-urgent medical inquiry or wish to schedule an appointment please do so through the myHealthSpot Patient Portal. If you are needing an immediate response, and it is outside of normal business hours, call the office and request the answering service put you in touch with the on-call provider.
